Check the Submission View tab for all of your quizzes.

If your course was migrated from BbLearn, there is a good chance that any quizzes or self-tests have NO default submission view. If that is the case, when a student completes a test and submits it, they will receive an internal error message. You must add a submission view to avoid this. Even in the case that a default view is there, the default may not be releasing the kind of information you want students to receive upon submission of their quiz.
